You may have to copy it and enlarge it so you can use it.
Instructions for using the form:
1. Use one line per item.
2. The last line item will indicate what Discounted Rate (B) to use.
3. Multiply each List Price (F) by the Discounted Rate (B) to get the
Discounted Price (G). ie: Assume your party purchased 18 items. The Discounted
Rate (B) for 18 items is .82, Therefore, you mulitply each List Price by .82
to derive each Discounted Price (G).
4. Add $2.50 (Processing fee) to shipping fees, if any, and divide the sum by
the number items purchased to derive the Processing & Shipping fees (H) for
each item ordered.
5. Add Discount Price (G) to Processing & Shipping fees (H) to derive each
item's Total Cost (I).
6. The sum of Total Cost (I) column should equal the total of your order.
7. Keep this list. You will need to track who has paid you and who has ordered
what items.
8. Also suggest that you print each order page (this is the page that has the
picture, description, and list price of the item being purchased) and write
the purchaser's name on it for your records.
